i've had this error using a modern pc with a ati video card. in my case it is related to a lack of available (video?) ram, caused by running wrptool while trying to load ofp. minimizing all programs and waiting around a second allows ofp to be started normally. wrptool can use as much as 600mb of ram. i'll learn how to wiki then add it.
